The Pennsylvania State Board of Pharmacy has modified its “Intern Experience Reporting Form.”  The new version of the “Intern Experience Reporting Form” may be downloaded at www.dos.state.pa.us/pharm under “Licensure Information”, “Board Forms.”  As always, we recommend that students and other applicants utilize this web site for the most current version of all applications.
