VAMC; WIlkes Barre

 The Wilkes-Barre VA Medical Center is one facility among ten within the VA Stars & Stripes Healthcare Network. Its mission is to maintain and improve veterans' health and quality of life, and to be a National Leader in the provision of health care services. The Wilkes-Barre VA Medical Center service area consists of 19 counties having a veteran population of over 212,000 that covers over 13,300 square miles. The Wilkes-Barre VA Medical Center is a General Medical and Surgical facility consisting of over 100 Operating Hospital Beds, 165 Nursing Home Beds, and 10 Substance Abuse Residential Rehabilitation Treatment program Beds. The facility serves veterans throughout northeastern and central Pennsylvania and southern New York State. The Wilkes-Barre has several outpatient-run clinics including the pharmacist managed medication management clinic, cardiovascular risk reduction clinic, and the anticoagulation clinic. 

Medication Management Clinic
 
The Medication Management Clinic (MMC) is a referral-based ambulatory care clinic designed to improve the drug use of geriatric VA patients. Functions of the clinic include:

    Reviewing and maximizing complex medication regimens

    Providing patient education

    Optimizing patient's disease therapy

 Typical consults include management of HTN, hyperlipidemia, diabetes mellitus, pharamcotherapy, polypharmacy and patient education. Provision of Care to VA patients is the goal of the clinic, and ultimate goal of the resident as a part of this clinic. The resident will be responsible for providing care to patients on a variety of illnesses, serving as a drug information specialist for other HC providers, provide educational inservices to medical residents and attendings, nurses and pharmacists, and participate in administrative functions of the hospital including but not limited to participation in the hospital P+T committee. 

Cardiovascular Risk Reduction Clinic
The Wilkes-Barre VAMC Cardiovascular Risk Reduction clinic is a pharmacist-run clinic designed to optimize care of patients with hyperlidemia and hypertension. The resident will become an integral part of the functioning of this clinic with the clinics main preceptors who are clinical pharmacists in the Wilkes-Barre VAMC pharmacy department.

Anticoagulation Clinic
 The Wilkes-Barre VAMC anticoagulation clinic is a pharmacist-run warfarin clinic designed to give optimal anticoagulation services to veterans. The resident will be an integral part of the functioning of this clinic with the clinic's main preceptors who are clinical pharmacists in the Wilkes-Barre VAMC pharmacy department.

Wyoming Valley Family Medicine Residency Program
The Wyoming Valley Family Practice Residency program consists of 18 medical residents (physicians-in-training), six physician faculty, seven nurses, one nurse practitioner, one psychologist, and one clinical pharmacist. The practice has approximately 20,000 outpatient visits per year.

Pharmacy Referral Clinic
The Pharmacy Referral Clinic (PRC), is a referral-based pharmacist-run ambulatory care clinic designed to help manage patients with complex medication regimens and improve the quality of patient care. Physicians and nurses may refer patients to the clinic for a number of disease states, including but not limited to:

HTN
Hyperlipidemia
Diabetes
Asthma
Smoking cessation
Patient education

Resident responsibilities will not only include providing direct patient care to a variety of patient populations but also include educating medical residents and faculty regarding drug therapy and evidence-based medicine during morning report conferences, serving as a consultant to all physicians for any medication related issues, obtaining medication for indigent patients through manufacturer patient assistance programs, and documenting teaching and clinical activities and interventions.
